The application of logistic regression method and ROC curve analysis in identifying risk factors of piperacillin-tazobactam induced allergic reactions
目的:探讨与抗菌药物哌拉西林他唑巴坦过敏反应有关的危险因素,为预防过敏反应的发生提供参考。方法:收集2012-2017年某院使用哌拉西林他唑巴坦发生过敏反应的病历59份作为试验组,并随机抽取同期使用该药未发生过敏反应的126份病例作为对照组,对致过敏的可能危险因素进行单因素分析、ROC曲线分析及多因素非条件Logistic回归分析。结果:单因素分析结果显示,患者年龄、过敏史、输液浓度、滴速等4个因素试验组与对照组之间的差异有统计学意义(P<0.05);ROC曲线分析结果显示,连续性变量患者年龄对是否发生过敏反应的预测价值有限,曲线下面积(AUC)为0.428,截断值78.5岁处敏感度为0.220,特异性为0.833;多因素非条件二元Logistic回归分析结果显示,患者过敏史、输液浓度、滴速为哌拉西林他唑巴坦致过敏反应的独立危险因素,而患者年龄不是该药致过敏反应的独立危险因素。结论:使用哌拉西林他唑巴坦过程中,应当加强致过敏反应相关的患者过敏史、输液浓度、滴速等危险因素的重视和干预,更好地预防过敏反应的发生。
OBJECTIVE To investigate related risk factors of piperacillin-tazobactam induced allergic reaction, and provide a reference for the prevention of the reaction.METHODS Totally 59 clinical medical records of piperacillin-tazobactam induced allergic reactions were collected in our hospital during 2012-2017 as experimental group; 126 medical records without piperacillin-tazobactam induced allergic reaction during the same period were included as control group. The single factor analysis, ROC curve analysis and multivariate logistic regression analysis were conducted for the potential risk factors of piperacillin-tazobactam induced allergic reaction.RESULTS Results of single factor analysis showed that there was statistical significance in 4 factors including the patient age, history of allergy, concentration of the preparation and dripping speed between experimental group and control group(P<0.05). ROC curve analysis indicated that the continuous variable patient age had limited predictive value in the incidence of allergic reaction, which resulting in a AUC 0.428 and a cutoff point age 78.5(sensitivity 0.22, specificity 0.833). Multivariate regression analysis showed that history of allergy, concentration of the preparation and dripping speed were independent risk factors to piperacillin-tazobactam induced allergic reaction, while the patient age was not the independent risk factor to piperacillin-tazobactam induced allergic reaction.CONCLUSION During the application of piperacillin-tazobactam, great importance should be attached and intervention should be carried out for the risk factors, including history of allergy, the concentration of preparation and dripping speed, so as to prevent allergic reactions effectively.
